STATEMENT OF NON-DISCRIMINATION
Home Telephone Co. is the recipient of Federal financial assistance from the U.S. Department of Agriculture (USDA). The USDA prohibits discrimination in all its programs and activities on the basis of race, color, national origin, religion, sex, gender identity (including gender expression), sexual orientation, disability, age, marital status, family/parental status, income derived from a public assistance program, political beliefs, or reprisal or retaliation for prior civil rights activity, in any program or activity of funded by USDA. (Not all prohibited bases apply to all programs.) The OASCR, through the Center for Civil Rights Enforcement, investigates and resolves complaints of discrimination in programs operated or assisted by USDA.
Persons with disabilities who require alternative means for communication of program information (Braille, large print audiotape, American Sign Language, etc.) should contact USDA’s TARGET Center at (202) 720-2600 (voice and TTY) or contact USDA through the Federal Relay Service at (800) 877-8339. Additionally, program information may be made available in languages other than English.
If you wish to file a Civil Rights Program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, found online at http://www.ascr.usda.gov/complaint_filing_cust.html, or at any USDA office, or call (866) 632-9992 to request the form. You may also write a letter containing all of the information requested in the form.
